A Regular Health Exam will be given for students beginning school in this October on the following date. Health exams help you stay healthy and detect disease early.

1. Target students
This health exam is only for students beginning school in this October (excluding research students, auditing students, special auditing students, non-degree students, and transfer students).


2. Place
University Hall – 4th Floor Reception hall　(On-campus, see campus map S1-5)


3. Health exam details
Filling in a medical questionnaire, measurement of height and weight, vision test, hearing test (by interview), measurement of blood pressure, chest X-ray test, and medical check by doctors

  　　　 13：30～16：00　Men
  

• You must have the following with you when you take the exam. Please have them ready.
  ・Student ID card　・※Health Observation Checklist　・Pencil/mechanical pencil and eraser　・T-shirt (with no buttons, metallic parts, prints) ← For chest X-ray test

• If you experienced fever or were in poor physical condition during the period from one week prior to taking the health exam to the day of your exam, you will not be able to take the exam. Additionally, please avoid visiting 3C settings (closed spaces with poor ventilation, crowded and close-contact settings), dining in large groups, or coming in contact with individuals who may be infected two weeks prior to the exam
• If you have a class scheduled during your assigned health exam time, please prioritize your class.
• The timing and place to pick up your health exam results (Individual Results Form) are as follows:
  Timing: Early December
  Place: Administration department of your college or graduate school
  

Off-campus Health Exams

If you are unable to take the health exam at your assigned time, you may take the off-campus health
exam provided free-of-charge by the Kanagawa Sogo Kenshin Center.

• You must make a reservation by telephone in advance (046-262-8155). We recommend that you make your reservation early because the health exams may be booked up ahead of time.
• If you experienced fever or were in poor physical condition during the period from two weeks prior to taking the health exam to the day of your exam, you will not be able to take the exam.
• You must take your student ID card with you on the day of the health exam.
• The timing and place to pick up your health exam results (Individual Results Form) are as follows:
  Timing: Four to five weeks after the health exam
  Place: Administration department of your college or graduate school
  

Health Examination Certificates

A Health Examination Certificate will be issued by the automatic certificate issuing system according to the following schedule

• Students who underwent an on-campus health exam
  
  From Tuesday, February 1, 2022
  


• Students who underwent an off-campus health exam
  Health Examination Certificates will be issued by the automatic certificate issuing system according to the following schedule. The data will be updated every two months.

If you need a Health Examination Certificate prior to the scheduled starting date for the issuing of the certificates, a certificate can be issued by the Kanagawa Sogo Kenshin Center (location of health exam) for a fee. Please inquire directly with the Kanagawa Labor Health and Welfare Association (046-262-8155) when making a reservation.
In addition, depending on who you will submit the document to, it may be possible to submit a copy of the Individual Results Form instead of a Health Examination Certificate. Please make inquiries and check the details yourself.

Other

• If you take a health exam at another medical institution, please submit the results to the University (administration department of your college or graduate school).


• Period for issuing Health Examination Certificates for 2021 academic year health exams using the automatic certificate issuing system
  
  We plan to make enable the issuing of certificates until May 2022
