About On-Campus Third Vaccination against COVID-19 (Booster Shots)(Update on June 13)

We at Yokohama National University (YNU) are going to conduct on-campus third vaccination against COVID-19 (Booster shots), thanks to Yokohama City University which will kindly send us a medical assistance team consisting of doctors, nurses, pharmacists, and other staff.
If you wish to receive COVID-19 vaccine booster shots on campus, read carefully the following instructions.


1.About COVID-19 vaccination


  • The on-campus vaccination is not compulsory or mandatory and will be given only with the consent of the person to be vaccinated after the information is provided. There is no penalty for those who opt out of vaccination. In addition, it does not prohibit you from receiving the vaccine at another site set by local governments, medical institutions, etc.
  • Vaccine available: Takeda/Moderna
  • Vaccination is free of charge.
  • Travel cost to the vaccination venue is to be paid at your own expense (please use public transportation as much as possible).
  • Please read and understand the advantages and side effects of COVID-19 vaccines before deciding to receive the vaccine.


2.Eligible persons


Those who have been vaccinated at least 6 5 months after the second COVID-19 vaccination and meet the following and wish to receive COVID-19 vaccination booster shots at YNU are eligible for application.(Persons 18 and over are eligible.)
※On May 25, 2022, the Ministry of Health, Labour and Welfare (MHLW) approved plans for people to receive their third and fourth shots of COVID-19 vaccines manufactured by U.S. pharmaceutical giants Pfizer Inc. and Moderna Inc. after a five-month interval, down from six months before. In light of this, the requirements have been changed.


 YNU students (both degree and non-degree students are eligible.)
 YNU faculty and staff (full-time, part-time, and temporary)
 Members of YNU-affiliated schools
 Contractors, security guards, cleaners and others who regularly work on the campus
 Students, teachers, and administrative staff of local schools
 Neighborhood residents and families of YNU students, faculty and staff

3.Vaccination Days


June 16 (Thu) - 17(Fri)
10:00 am to 12:30 pm, 2:00 pm to 4:30 pm

* To ensure that no vaccines go to waste, choose a date and time to be surely able to receive the vaccine unless you are sick or encounter unexpected matters.


4.How to make an appointment


* When making your reservation, you will need the vaccination number and municipality code found in the vaccination voucher sent to you by your municipality.
**Online appointments will start at 9:00 am on Monday, June 6, 2022.
Please apply by the day before the vaccination date.(The reservation deadline has been extended.)
***If you have not yet received your vaccination voucher, please contact the call center.
Please also contact the call center for any changes or cancellations on the day of the vaccination.

5.What to bring on the day

① "Prevaccination Screening Questionnaire for COVID-19 Vaccine"

* If the Prevaccination Screening Questionnaire for COVID-19 Vaccine sent by the municipality is a copy-type document, please bring it with you without peeling off the second and subsequent pages.
** Please be sure to fill out the Prevaccination Screening Questionnaire for COVID-19 Vaccine and bring it with you.

② "Vaccination Voucher," "Certificate of Vaccination"

* Forms vary by municipality, so if you are unsure, please bring the entire envelope with you.


③ Identity verification document

* Please bring an official document that confirms the address where your resident registration is registered. (e.g., driver's license, Individual Number Card, residence card, health insurance card, etc.)
** If you do not have one of these, please bring your student or faculty/staff ID card.

【Notes】

○ Vaccination requires the Vaccination Voucher sent by the local government, so please be sure to prepare it in advance. If you do not have the Vaccination Voucher on the day of the vaccination, you cannot be vaccinated.

○ If you have lost your Vaccination Voucher or moved to a new address after the 2nd vaccination, please apply for reissuance of the Vaccination Voucher to the municipality in which you reside.

○ If your identity cannot be confirmed, please note that you cannot get vaccinated.

8.Vaccine-related Absences (for YNU students and faculty and administration staff)

Safety must come first. Absences for those who suffer side effects should be excused.
